I would be very useful a "Add heading button" or just a "+" next to "Add custom code" and "Add shortcode") between heading in the outline.

Usually, when I want to manually add a heading, I have to go to the top , write the heading, click en "+" button, take the heading from the bottom and place it were it belongs, when adding several manual headings this is time consuming and not straightforward.
